Usman Shinwar (4-15) and Umer Khan (3-22) helped Karachi Kings restrict Multan Sultans to a modest total of 118-7 before Liam Livingstone (53*) struck his third fifty of the season to take his side home with four balls to spare and five wickets in hand.
Chasing a total of 119, Karachi lost their openers for 36 runs, both by Mohammad Ilyas, before Liam Livingstone and Colin Ingram added 46 runs for the third-wicket partnership to take Karachi close. But a flurry of wickets and slow scoring rate saw Karachi drag the game to the final over. When Mohammad Rizwan departed on the last ball of the penultimate over, Karachi were left with seven runs to get in the last over.
Multan chose Mohammad Irfan Khan to bowl the last over, having bowled out their frontline seamers: Mohammad Abbas (2-14), Mohammad Irfan (0-20) and Mohammad Ilyas (3-27). Liam Livingstone fetched two runs off the first ball before belting the next one over deep midwicket to seal the game for Karachi.
Karachi have now moved to the fourth spot with eight points in as many games with four wins and losses each.
Earlier, Multan struggled to kick on after being put into bat first, continuously losing wickets. Usman Shinwari started the carnage at the top before Umer Khan chipped in. Only Shoaib Malik (26 off 26) and Hammad Azam (29 off 30) could put some resistance.
